Four Steps Required before the Initial Survey

If CMS approves an initial hospice certification survey, HHSC requires agencies to complete four prerequisite steps before it conducts an initial survey.

These four steps must be completed within six months of the date of the initial license:

• Complete the state licensing process.
• Complete form CMS-855A and submit to CMS.
• Be operational.
• Request an initial survey.
