2. Complete form CMS-855A and submit to CMS.

The hospice must apply to CMS for certification by completing and submitting Form CMS-855A (Application for Certification).  

It is the hospice's responsibility to ensure the CMS-855A is current. The CMS-855A is valid for six months. For more information, see RS&C Letter #06-04 .
