In order to comply with 40 TAC §97.405(n), agencies providing home dialysis services must develop a long-term program for each dialysis client with the client and interdisciplinary team.

Criteria must be defined in writing and must provide guidance to the agency in the selection of clients suitable for home staff-assisted dialysis and in noting changes in a client's condition that would require discharge from the program.

Agencies providing home dialysis services must ensure that each client has a timely physical examination and history performed and updated at least annually after the date of admission.
