As required by 40 TAC §97.405(r), medications can only be administered by licensed personnel.

Records of home staff-assisted dialysis clients must include the following (in addition to the applicable information described in 40 TAC §97.301(a)(9) ):

• a medical history and physical;
• clinical progress notes by the physician, qualified licensed nurse, qualified dietitian, and qualified social worker;
• dialysis treatment records;
• laboratory reports;
• a client care plan;
• a long-term program; and
• documentation of supervisory visits.
